Services & Programs

 Chrysalis Community Partners provides a comprehensive range of services and programs designed to support justice-involved individuals as they transition from custody back into the community. 


Our approach focuses on addressing the barriers that often contribute to instability, including housing challenges, untreated behavioral health needs, substance use disorders, limited employment opportunities, and lack of coordinated support systems.


Our services may be delivered directly, through partnerships, or through coordinated referral networks, depending on the needs of the individual, organization, or community partner.

Collaborative Case Management

  Our case management model is designed to provide each participant with a personalized plan for success. CCP works to identify individual risks, needs, strengths, barriers, and goals.


Services may include:

Personalized assessments
Individualized service plans
Coordination with criminal justice agencies
Connection to housing resources
Employment readiness and job placement support
Education and vocational guidance
Behavioral health and substance use referrals
Monitoring of progress and compliance
Warm handoffs to community providers


Our goal is to help participants move from crisis and instability toward structure, accountability, recovery, and long-term independence.

Reentry Continuity of Care / MAT OUD Program

  One of CCP’s core priorities is preventing post-release clinical drop-off. The transition from custody to community is a high-risk period, especially for individuals with substance use disorders, chronic medical needs, or behavioral health conditions.

CCP supports reentry continuity of care by helping coordinate:

Medicaid or insurance activation when eligible
Medical and behavioral health appointments
Medication continuity
MAT provider linkage for Opioid Use Disorder
Transportation planning
Care plan coordination
Warm handoffs to community providers
Follow-up and accountability after release

For individuals with Opioid Use Disorder, continuity of care is especially critical. CCP supports pathways that reduce treatment gaps, relapse risk, overdose risk, and preventable crises.

Housing, Nutrition & Stability Support

  Stable housing is one of the strongest foundations for successful reentry. CCP’s model includes safe, structured, drug-free housing support and connection to resources that help individuals stabilize after release.

Where applicable, CCP also supports access to nutritious meals, basic needs, and daily structure so participants can focus on recovery, employment, education, and personal responsibility.

Education & Employment Readiness

  CCP recognizes education and employment as essential tools for reducing recidivism and improving long-term stability.


Program supports may include:

GED or high school equivalency support
Basic literacy and numeracy support
Financial literacy
Life skills development
Job readiness training
Resume and interview preparation
Vocational training pathways
Employer and workforce partner coordination

Behavioral Health, Substance Use & CBT-Based Programming

  CCP supports evidence-based behavioral change by connecting participants to services and programming that address addiction, criminal thinking patterns, emotional regulation, decision-making, and personal accountability.


Programming may include:

Cognitive behavioral interventions
Substance use treatment coordination
Mental health service linkage
Prosocial modeling
Life skills groups
Relapse prevention support
Recovery planning

Entrepreneurship & Workforce Development

  CCP believes economic opportunity can help individuals rise above stigma and build productive lives. Our entrepreneurship and workforce development focus helps participants explore pathways to business ownership, skilled labor, construction, public works, and long-term employment.

Entrepreneurship education
Mentorship
Job readiness
Construction skills pathways
Financial literacy
Business concept development
Employment partner referrals
